Go to miretirement.com or contact BMO Global Asset Management (formerly known as M&I Institutional Trust Services) at (800) 858-3829, available 24/7 to enroll online. Or you may complete a 401(K) Election Form. The completed form must be provided to Local 49 and a copy must also be provided to Southwest Service Administrators Inc. (SSA).

What happens to my money if I die? 
Your account balance will be distributed to your designated beneficiary when they apply for benefits. If you do not have a designated beneficiary, any money in your account will be distributed to your heirs or to your estate in accordance with the law. Payments may be delayed if no beneficiary has been designated. 
 
How do I designate or change my beneficiary? 
Go to miretirement.com or contact BMO (fka M&I) at (800) 858-3829, available 24/7 to complete a Designation of Beneficiary form.
 
How do I change my address? 
Complete and print the Online Change of Address Form and return it to the Fund Office: Southwest Service Administrators Inc. 2400 W. Dunlap Avenue, Suite 250 Phoenix, AZ 85021  
 
When can money be withdrawn from my 401(K) account? 
1. At Normal Retirement (age 65 or the 5th anniversary of participation in this plan) as long as you refrain from employment of more than 39 hours per month in the sheet metal industry in the geographic area of the plan.
2. When you terminate employment & do not work for a contributing employer for a period of one Plan Year (April 1 – March 31).
3. When you reach age 55, terminate employment, and refrain from employment for a contributing employer.
4. When you become totally & permanently disabled and receive Social Security Disability benefits
5. When you die and your beneficiary applies for benefits.
6. When you apply and qualify for a Hardship Withdrawl.
